SWEENY ISD REQUIRES THAT EACH ATHLETE HAVE AN ANNUAL PHYSICAL DATED AFTER MAY 17TH
PHYSICALS MAY 17, 2022 FROM 5:30 to 7:30
Anybody who is planning on playing a sport, being in the band, drill team, or cheerleading during the 2022-2023 School year will need to get a physical before they are allowed to participate in those extra-curricular activities including practices.

Our district requires all extra-curricular activities have a new physical on file every year.

All students participating in UIL marching band and/or UIL athletic activities will be required to complete a new physical for the upcoming year.

All incoming 7th graders and any NEW High School marching band member must receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches.

Any participant in athletics or marching band, in any grade level, who has not previously completed a Pre-Participation Physical Evaluation (PPE), and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.

Concussion Acknowledgement Form

The UIL has created this Concussion Acknowledgement Form, which will be required for all student athletes in grades 7-12 beginning with the 2020-21 school year, as a result of the passage of HB 2038 from the 2011 legislative session.

According to section 38.155 of the Texas Education Code, 'a student may not participate in an interscholastic athletic activity for a school year until both the student and the student ’s parent or guardian or another person with legal authority to make medical decisions for the student have signed a form for that school year that acknowledges receiving and reading written information that explains concussion prevention, symptoms, treatment, and oversight and that includes guidelines for safely resuming participation in an athletic activity following a concussion…..'
